What a complete trucking submission needs.

Start with a legal business name, USDOT and authority details, operating radius, states, commodities, annual mileage, requested effective date, vehicle schedule, driver schedule, and available loss history. Add contracts or certificate requirements that could change requested limits or endorsements.

Vehicle schedule essentials

  • Year, make, model, VIN, stated value, ownership, and loss payee
  • Trailer type, value, refrigeration equipment, and permanent modifications
  • Garaging location and expected operating territory

Driver schedule essentials

  • Full name, license state and number, date of birth, and hire date
  • Relevant commercial experience and recent violations or accidents
  • Hiring, MVR, drug testing, and ongoing monitoring procedures
